Job summary

Change Incorporated is seeking a compassionate Certified Nursing Assistant to provide in-home care in Torrington, CT. You will promote independence, safety, and well-being for clients in their own homes, following individualized care plans.

The role involves assisting with daily activities, coordinating with a Care Supervisor, and supporting emotional and social needs. Competitive hourly pay and a flex schedule are offered.

Qualifications

• High school diploma or equivalent.
• Active CNA certification in Connecticut or eligibility to transfer to Connecticut.
• Ability to follow care plans, prioritize tasks, and work effectively in a team environment.
• Physical stamina to lift and assist individuals safely (25+ lbs) and perform duties during long shifts.

Responsibilities

• Perform activities to help participants with meals, dressing, grooming, and hygiene.
• Assist or cue participation in family, social, and community activities.
• Support mobility and safe transfers between beds, wheelchairs, and other locations.
• Provide emotional support to individuals and families and help maintain independence.

Certified Nurse’ Aide

FLSA Status: This position is considered NON-EXEMPT in relation to the pay provisions of the Fair Labor Standards Act. The work schedule is a “flex schedule” in that it may require work 1st, 2nd, or 3rd shift based on the needs of the individuals served by Change Incorporated. This position may be assigned to work in various locations/homes. If working hours extend beyond normal business hours, compensation will follow wage and hour guidelines established under the Fair Labor Standards Act as identified in the Change Inc’s Personnel Policies and Procedures.

Purpose:

As a dedicated Certified Nursing Assistant (CNA), your primary purpose is to provide compassionate, personalized care that promotes independence, safety, and well-being for our clients in their own homes. You will play a vital role in enhancing quality of life by delivering respectful assistance with daily activities, fostering meaningful connections, and supporting their physical and emotional needs.

Essential Responsibilities:

1. Performing the following tasks if the participant (by reason of physical or psychiatric disability) is unable to perform them, or assisting, or cueing the participant to perform them:
   • Meal planning and preparation, shopping, housekeeping (e.g., changing linens, washing dishes, vacuuming/ dusting, laundry, mending clothing repairs), basic household tasks (e.g., regulating home temperature, storing food appropriately, resolving issues about bill paying).
   • Dressing, personal grooming, bathing, and hygiene.
   • Appropriate use of emergency medical services.
2. Assisting or cueing the participant to perform or become engaged in:
   • Family, social, and recreational activities.
   • Appropriate use of natural community supports (e.g., social clubs, faith-based supports).
   • Appropriate use of routine medical/ dental services
   • Use of medications as prescribed, including self-administration of medications.
   • Healthy habits (e.g., healthy diet, exercise, and behaviors designed to alleviate stress).
   • Fulfillment of personal commitments, and adherence to scheduled appointments/ meetings (e.g., clinical, vocational, educational, and judicial/court)
3. Supportive and compassionate emotional support to individuals and their families.
4. Supporting mobility efforts, including transferring patients between beds, wheelchairs, and other locations.
5. Establishing and maintaining a helpful, supportive, companionship relationship with the participant that involves such activities as:
   • Escorting the participant to necessary medical, dental, or personal business appointments
   • Reading to or for the participant
   • Engaging in or discussing recreational, hobby, or sport-related activities
6. Other activities directed at restoring participant functioning, and achieving independent participation in social, interpersonal, family, or community activities and full community re-integration and independence.

Required Skills, Education, Training:
