Job summary

Interim HealthCare - Great Lakes Health Partners is seeking a Hospice/Home Health Certified Nursing Assistant (CNA) in Rochester, MN and surrounding areas. You will provide one-on-one home care, helping with activities of daily living and monitoring patient condition, while building meaningful relationships with patients and families.

Qualifications

• Must have MN CNA certification active.
• CPR certification required.
• Valid driver’s license, auto insurance and transportation.
• Compassionate nature and good communicator.

Responsibilities

• Care for seniors and those with an illness, injury or chronic disease in patients' homes.
• Assist with bathing, toileting, dressing, grooming, hygiene, nutrition.
• Assist with ambulation, transfers and ROM exercises.
• Check vitals, intake/output, glucose, and document changes.
• Collaborate with Hospice/Home Health nurses to follow care plans.
• Observe patient and notify supervisor of issues needing changes in care.

As a Hospice/Home Health Certified Nursing Assistant, heres a big-picture view of what youll do:

• Care for seniors and those with an illness, injury or chronic disease in their homes
• Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
• Assist with ambulation, transfers and range of motion exercises
• Check vitals, measure intake/output, monitor blood glucose and document any changes
• Work with a team of Hospice or Home Health nurses to ensure a patients care plan is followed
• Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment

A few must-haves for Hospice/Home Health Certified Nursing Assistants:

• High school diploma (or equivalent) and active Nursing Assistant Certification in Minnesota
• CPR certification
• Valid drivers license, auto insurance and transportation
• Compassionate nature, good communicator and ability to lift 50 lbs.
