Job summary

Interim HealthCare of Rochester is hiring Hospice/Home Health CNAs to provide one-on-one care to patients in their homes across Rochester, MN and surrounding areas. The role focuses on compassionate, patient-centered support, with no nights or weekends and flexible scheduling.

Mileage reimbursement accompanies a strong team-based approach with nurses to implement care plans. Applicants must hold a Minnesota CNA certification, CPR, and a valid driver’s license; high school diploma or equivalent

Qualifications

• High school diploma (or equivalent) and MN CNA certification.
• CPR certification is required.
• Valid driver’s license and reliable transportation.
• Compassionate, good communicator, and able to lift 50 lbs.

Responsibilities

• Care for seniors and homebound patients in their homes.
• Assist with daily activities (bathing, toileting, dressing, grooming, nutrition).
• Check vitals and monitor changes; document observations.
• Work with Hospice/Home Health nurses to follow the care plan.
• Observe patients and report changes to supervisor or medical staff.
