Job summary

CareLinx by Sharecare is hiring Certified Nursing Assistants (CNAs) in Thousand Oaks, CA to provide personal care and clinical support across care settings. New graduates are welcome, and you set your availability via the CareLinx app to match with clients.

You will work with a Care Advisor team to find ideal matches based on location and needs, maintaining professionalism, privacy, and patient safety at all times.

Qualifications

• Active CNA certification or license.
• Active CPR/BLS certification.
• Minimum 2 months of nursing assistance experience.
• Proof of COVID vaccination including first booster.
• Access to a smartphone capable of running the CareLinx app.
• Ability to communicate in English (bilingual encouraged).
• Reliable transportation to and from shifts.
• Must pass a background check.

Responsibilities

• Assist patients with personal care including bathing, dressing, feeding, toileting, ambulation, transfers, and positioning.
• Aid with physician-ordered medications under RN direction.
• Ensure patient privacy, dignity, and safety in a secure environment.
• Document all care tasks in the CareLinx mobile app during each shift.
• Submit timesheets on time through the CareLinx app.
• Communicate with Care Advisor regarding case updates or changes in patient condition.
• Maintain client confidentiality at all times.
• Additional shift opportunities based on client needs (transportation, discharge escort, telehealth navigation).

WHAT YOU'LL DO

Core Responsibilities:

• Assist patients with personal care including bathing, dressing, feeding, toileting, ambulation, transfers, and positioning
• Aid with physician-ordered medications that are ordinarily self-administered, under the direction and supervision of a Registered Nurse
• Ensure patient privacy, dignity, and well-being in a safe and secure environment
• Document all care tasks accurately in theCareLinxmobile appduringeach shift
• Submit timesheets on time through theCareLinxapp
• Communicate with your Care Advisorregardingany case updates or changes in patient condition
• Maintain client confidentiality at all times

Additional shift opportunities (based on client needs):

• Transportation and errand support
• Hospital discharge escort — safelyaccompanyingmembers home after discharge
• Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required Qualifications:

• Active CNA certification or license
• Active CPR/BLS certification
• Minimum 2 months of nursingassistanceexperience
