Job summary

Elara Caring is seeking a Certified Nurse Aide to join our home health team. You will provide direct patient care under a Registered Nurse’s supervision, helping with daily living activities and documenting care.

Responsibilities include vital signs, mobility assistance, and following individualized care plans while maintaining safety and infection control standards in patients’ homes.

Qualifications

• Current State certificate as a Certified Nursing Assistant.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes.

• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient’s individualized care plan.
• Monitor and report changes in a patient’s condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation and protect patient privacy.

What is Required?

• Current State certificate as a Certified Nursing Aide.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
