Job summary

Elara Caring is hiring a Certified Nurse Aide to provide in-home care and support to patients in their own homes across Indiana. You will follow individualized care plans and work under the supervision of a Registered Nurse to deliver essential daily living activities with compassion.

You will monitor patient conditions, document accurately, and ensure safety and infection control standards while traveling between patient homes.

Qualifications

• Provide hands-on personal care under direction of a Registered Nurse.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient’s individualized care plan.
• Maintain accurate documentation and patient privacy.

As a Certified Nurse Aide , you’ll contribute to our success in the following ways:

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient’s individualized care plan.
• Monitor and report changes in a patient’s condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
