Job summary

Elara Caring in Louisiana is seeking a Certified Nurse Aide to join our home care team. You will provide hands-on personal care under supervision, assist with daily activities, and monitor patient condition to support rapid, compassionate care in the home.

The role requires a state-certified CNA or Home Health Aide certificate, ability to lift up to 50 pounds, and reliable transportation to travel between patient homes.

• Must possess valid drivers license or reliable transportation to travel between patient homes

Responsibilities

• Provide hands-on personal care under RN supervision
• Assist patients with bathing, dressing, grooming, toileting, and activities
• Take and record vital signs including temperature, pulse, respirations, and blood pressure
• Help patients walk, transfer, and use mobility equipment
• Follow each patients care plan and report changes to the care team
• Maintain a clean, safe home environment and protect patient privacy

Job Description

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
