Job summary

Ascension's Franciscan Place in Brookfield, WI is seeking a Nurse Aide to join the Long term Care & Skilled Nursing team. The role offers an afternoon shift with every other weekend and full-time scheduling, providing essential resident care and support.

You will assist with daily living activities, support clinical tasks, monitor residents, and help maintain a safe, compassionate environment. A Nurse Aide certification and BLS are preferred; a high school diploma or GED is common.

Qualifications

• Nurse Aide certification required prior to hire date.
• BLS (AHA or Red Cross) preferred.

Responsibilities

• Provide essential care: assist residents with daily living activities, personal care and mobility.
• Assist nursing team with procedures and maintain a clean, organized environment for healing.
• Monitor resident condition and report vital changes to the RN.
• Identify safety hazards and respond to emergencies to ensure a safe living environment.

How you’ll make an impact in this role

• Provide Essential Care: Support residents with daily living activities, personal care, and mobility, ensuring they feel comfortable, respected, and well-cared for.
• Support Clinical Success: Assist the nursing team with physician-ordered procedures and maintain a clean, organized environment for optimal healing.
• Be the Eyes and Ears: Monitor and report vital changes in a resident’s physical or emotional condition to the RN, playing a key role in early intervention.
• Champion Resident Safety: Proactively identify safety hazards and respond to emergency situations to ensure a secure and supportive living environment.

What Minimum Requirements You’ll Need

• Nurse Aide obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education

• High school diploma or GED preferred.
