Job summary

Unity Hospice is seeking a Certified Nursing Assistant (CNA) to provide in-home and community-based care under the supervision of an RN. You will perform ADL tasks, mobility assistance, and vital sign checks while ensuring compassionate, quality care for patients in their homes and facilities.

The role involves cross-training for hospice residence support, with a flexible schedule including weekends, on-call and holiday shifts. A valid Wisconsin CNA license and reliable vehicle are required.

Qualifications

  • Must be listed on the Wisconsin Nursing Assistant Registry.
  • Valid driver’s license and access to a reliable vehicle.

Responsibilities

  • Provide activities of daily living support.

The Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN’s comprehensive assessment of the patient’s personal care needs and are documented in the Activities of Daily Living Plan of Care. This is a community-based position in which the CNA travels to patients’ homes and various care facilities to provide individualized personal care and support. The CNA works independently in the community while maintaining communication and collaboration with the Unity RN and care team to ensure patients receive safe, compassionate, and appropriate care.

During times when census is higher, you will be cross-trained into our hospice residence and may support the team by providing care to patients at our Meng facility. This flexible approach helps ensure consistent, high-quality care across all settings as patient needs fluctuate.

Schedule:

Full-time, 0.9 FTE, consisting of three 12-hour shifts per week (6:00 a.m.–6:30 p.m.), with every third weekend rotation. The position also includes approximately 10–15 on-call shifts per year and 2–3 holiday shifts per year.
