Job summary

Unity Hospice WI is seeking a Certified Nursing Assistant (CNA) to provide compassionate, patient-centered care in home and facility settings. This community-based role requires travel to patient homes under RN supervision and cross-training to the Meng facility when needed.

The CNA will support ADLs, mobility, nutrition, vital signs, and teaching; collaborate with the care team to ensure safe, high-quality care across settings.

Qualifications

  • Completion of certified nursing assistant program.
  • Must be listed on the Wisconsin Nursing Assistant Registry and be eligible to work at state and federally certified facilities.
  • Valid driver’s license, auto insurance, and access to a reliable vehicle.

Responsibilities

  • Provide personal care and support under RN supervision.
  • Travel to patients' homes and care facilities to deliver care.
  • Maintain communication with the Unity RN and care team.
  • Cross-train to hospice residence when census requires.
  • Assist with activities of daily living and mobility.
  • Monitor vital signs and report changes to the team.
  • Support families and provide companionship.
  • Order supplies and ensure patient comfort.
  • Maintain required competencies and skills.

Description

Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN’s comprehensive assessment of the patient’s personal care needs and are documented in the Activities of Daily Living Plan of Care. This is a community-based position in which the CNA travels to patients’ homes and various care facilities to provide individualized personal care and support. The CNA works independently in the community while maintaining communication and collaboration with the Unity RN and care team to ensure patients receive safe, compassionate, and appropriate care. During times when census is higher, you will be cross-trained into our hospice residence and may support the team by providing care to patients at our Meng facility. This flexible approach helps ensure consistent, high-quality care across all settings as patient needs fluctuates.

Schedule

Full-time, 0.9 FTE, consisting of three 12-hour shifts per week (6:00 a.m.–6:30 p.m.), with every third weekend rotation. The position also includes approximately 10–15 on-call shifts per year and 2–3 holiday shifts per year.
