Job summary

Unity-Hospice is seeking a Float B Registered Nurse for the NOC shift to cover Triage, Unity Hospice Residence, and Community patients. The role works with the care team to maintain high-quality patient care and serves as a key link among services.

The position is full-time at 0.8 FTE, with two 12-hour shifts and one 8-hour shift, including every other weekend and 10–15 on-call shifts yearly. On-site at Unity Hospice with comprehensive benefit support.

Responsibilities

  • Provide skilled nursing assessment and appropriate intervention.
  • Update the plan of care and review with patient and family.
  • Respond to urgent patient needs with triage nursing staff.
  • Initiate crisis care as needed.
  • Report findings to primary team and attending physician.
  • Educate patient/family on disease progression and symptom management.
  • Administer and titrate medications per orders.
  • Manage medications and Residence contingency meds.
  • Record orders in the EMR and follow up as needed.
  • Prioritize daily work and coordinate with Unity and staff.
  • Communicate with interdisciplinary team members.
  • Delegate nursing duties to LPN and supervise CNA care.
  • Complete new patient admissions as needed.
  • Triage calls and follow-up on care related contacts.
  • Document patient and physician communications.
  • Fax/call prescriptions per orders.
  • Coordinate volunteer deliveries.

Description
Float B Registered Nurse - NOC shift

The Registered Nurse - Float B is responsible for rotating duties covering Triage, Unity Hospice Residence, and Community patients. The nurse works collaboratively with the primary care teams, interdisciplinary team, and schedule coordinators to achieve high-quality patient standards.

Schedule:

This is a full-time position, working 32 hours per week as a 0.8 FTE, consisting of two 12-hour shifts and one 8-hour shift. The 12-hour shifts are 7:00 PM–7:30 AM, and the 8-hour shift is 11:00 PM–7:00 AM. You will be trained to support Triage, Hospice Residence, and Community services, with the Hospice Residence serving as the home base for all shifts. The schedule includes working every other weekend from 7:00 PM–7:00 AM, participating in an on-call rotation of approximately 10–15 shifts per year, and working two holidays per year.

Key Job Duties:
  1. Provide skilled nursing assessment and appropriate intervention
  2. Update the plan of care as necessary and review care plans with patient and family
  3. Respond to urgent patient/family needs in collaboration with triage nursing staff
  4. Initiate crisis care as needed
  5. Report appropriate findings and significant changes to the primary team, attending physician and primary caregiver
  6. Provide education to patient/family members on disease progression, symptom management, medication and other associated patient care needs
  7. Administer and titrate medication according to physician/nurse practitioner orders
  8. Mange individual patient medication and Residence contingency medications
  9. Record all received orders to the Electronic Medical Record and initiate follow up as needed
  10. Prioritize daily work and make adjustments taking into account the needs of patients, Unity and other staff
  11. Communicate with interdisciplinary team members
  12. Delegate nursing duties to LPN and supervise care provided by CNA
  13. Complete new patient admissions as needed
  14. Telephone triage and follow-up to patient care related contacts
  15. Record and maintain complete and accurate records of patient and physician communications
  16. Fax/call new and refill prescriptions according to physician orders
  17. Coordinate volunteer deliveries

Requirements
Minimum Qualifications & Certification/Licensure:
  1. Associate degree in Nursing required, bachelor’s degree preferred.
  2. RN license in the state of practice required (RN must practice under the law and regulations governing nursing in the state of practice).
  3. Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage.
