Job summary

Unity Hospice WI in De Pere seeks a PRN Certified Nursing Assistant for PM/NOC shifts at Meng Residence. The CNA provides delegated patient care under an RN and supports daily living activities in a hospice setting.

Shifts run 6:00 pm–6:30 am with minimal weekend/holiday rotation; cross-training may occur and willingness to cover day, PM, and night as needed. Must be listed on the Wisconsin CNA Registry and have a valid driver's license.

Qualifications

  • Completion of certified nursing assistant program.
  • Must be listed on the Wisconsin Nursing Assistant Registry and be eligible to work at state and federally certified facilities.
  • Valid driver’s license, auto insurance, and access to a reliable vehicle.

Responsibilities

  • Provide services delegated and supervised by the Unity RN based on the patient’s plan of care.
  • Assist with Activities of Daily Living, Mobility, Restorative Care, Skin Care, and Nutritional Support.
  • Monitor Vital Signs, read CADD pump settings, and report pain levels and changes to the interdisciplinary team.
  • Reinforce patient teaching, provide companionship, maintain confidentiality, and ensure 24/7 coverage as needed.

PRN Certified Nursing Assistant - PM/NOC Shift at our Meng Residence in De Pere

The Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN's comprehensive assessment of the patient’s personal care needs and are documented in the Activities of Daily Living Plan of Care.

Schedule

This PRN Pool 1 opportunity requires a minimum commitment of 16 hours per month with shifts that are 6:00 pm - 6:30 am. There is no regular weekend or holiday rotation; however, flexibility to pick up shifts that fall on weekends or holidays is expected. Cross-training to the community may be required, and candidates should be willing to work day, PM, and night shifts as needed.

Minimum Qualifications & Certification/Licensure
  • Completion of a certified nursing assistant program
  • Must be listed on the Wisconsin Nursing Assistant Registry and be eligible to work at state and federally certified facilities
  • Valid driver’s license, auto insurance, and access to a reliable vehicle

  • Computer proficiency (MS Office – Word, Excel, and Outlook)
  • Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems.
  • Ability to communicate effectively, both verbally and in writing.

Essential Job Duties And Responsibilities

The Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned and supervised by a Unity RN.   • Report any changes in the patient’s condition or family situation immediately to the appropriate member(s) of the interdisciplinary team
  • Promote and maintain confidentiality
  • Shares in providing 24-hour, seven day-a-week coverage
