Unity Hospice is seeking a Certified Nursing Assistant (CNA) for community-based care in Green Bay, WI. The CNA provides delegated patient care under the supervision of an RN, traveling to patients’ homes and care facilities to deliver compassionate support.
Responsibilities include ADL support, restorative care, and monitoring vital signs, with cross-training for hospice residence coverage as needed. Full-time, 0.9 FTE, with weekend rotation and eligible benefits.

The Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN’s comprehensive assessment of the patient’s personal care needs and are documented in the Activities of Daily Living Plan of Care. This is a community-based position in which the CNA travels to patients’ homes and various care facilities to provide individualized personal care and support. The CNA works independently in the community while maintaining communication and collaboration with the Unity RN and care team to ensure patients receive safe, compassionate, and appropriate care.
During times when census is higher, you will be cross-trained into our hospice residence and may support the team by providing care to patients at our Meng facility. This flexible approach helps ensure consistent, high-quality care across all settings as patient needs fluctuate.
Full-time, 0.9 FTE, consisting of three 12-hour shifts per week (6:00 a.m.-6:30 p.m.), with every third weekend rotation. The position also includes approximately 10-15 on-call shifts per year and 2-3 holiday shifts per year.
