Job summary

Nuclear Care Partners is seeking a licensed CNA for in-home care in Cedaredge, CO. You will support patients with daily living activities, provide companionship, and help with meals and light housekeeping in a home setting.

The role requires active Colorado CNA certification and CPR, a valid driver's license, and the ability to work independently. Home health or hospice experience is preferred; travel to nearby towns is expected.

Qualifications

Responsibilities

About the Role:

At Nuclear Care Partners, we provide compassionate, in-home care for individuals impacted by their work in the Department of Energy or Nuclear Energy programs. As a CNA, you'll play a vital role in supporting our patients' quality of life through consistent, one-on-one, relationship-based care in their homes. Compensation:

$1/hr shift differential for nights and weekends

• PTO (Full-Time and Part-Time employees only)
• Medical, Dental, and Vision (Full-Time only)
• Employee Assistance Program (EAP)
• 401(k) Option

Shifts:

• Travel to Montrose, Delta, and Cedaredge
• Full-Time - 30 plus hours a week
• Part-Time - less than 30 hours per week
• PRN - minimum of 12 hours every month

Key Responsibilities:

• Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility)
• Provide companionship, emotional support, and social engagement
• Support meal preparation, nutrition support and light housekeeping
• Monitor and report changes in condition
• Follow care plans and document accurately using EMR standards
