Job summary

Nuclear Care Partners is seeking a dependable CNA to deliver compassionate, in-home care in Harlem, Georgia. You will work in patients' homes providing one-on-one, relationship-based support to individuals who served in energy programs.

Responsibilities include ADLs, companionship, meal prep, light housekeeping, and documenting care using EMR. Must have GA CNA, CPR, HS diploma, driver's license. We offer PTO, benefits and 401(k).

Qualifications

• High school diploma or GED required.
• Active GA CNA certification required.
• CPR certification required.
• Valid drivers license and reliable transportation.
• Experience in home health, long-term care, or hospice preferred.

As a CNA, you'll play a vital role in supporting our patients' quality of life through consistent, one-on-one, relationship-based care in their homes. Compensation:

Compensation is based on experience, shift type, and market factors. The listed range reflects entry-level to highly experienced applicants.

Shifts: Full-Time Day shifts with alternating weekends

Key Responsibilities:

• Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility)
• Provide companionship, emotional support, and social engagement
• Support meal preparation, nutrition support and light housekeeping
• Monitor and report changes in condition
• Follow care plans and document accurately using EMR standards

Qualifications:

• High school diploma or GED
• Active CNA certification in GA
• CPR certified
• Valid driver's license and reliable transportation
• Ability to work independently in a non-facility setting
• Home health, long-term care, or hospice experience preferred.
