Job summary

Nuclear Care Partners is seeking a compassionate Part-Time CNA for in-home, one-on-one patient care in Nucla, CO. You will support ADLs, provide companionship, assist with nutrition and light chores, and follow care plans with EMR documentation.

Active Colorado CNA certification and CPR are required, along with a valid driver’s license and reliable transportation. Prior home health or hospice experience is preferred.

Qualifications

• High school diploma or GED.
• Active Colorado CNA certification.
• CPR certification.
• Valid driver's license and reliable transportation.
• Ability to work independently in a non-facility setting.
• Home health, long-term care, or hospice experience preferred.

Responsibilities

• Assist with ADLs (bathing, dressing, grooming, toileting, mobility).
• Provide companionship and emotional support.
• Support meal preparation and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document using EMR standards.

Key Responsibilities

• Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility)
• Provide companionship, emotional support, and social engagement
• Support meal preparation, nutrition support and light housekeeping
• Monitor and report changes in condition
• Follow care plans and document accurately using EMR standards
