Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our care team. This role involves hands-on personal care, comfort, and support for hospice patients in homes and facilities. You will work with an interdisciplinary team to maintain dignity and quality of life.

We offer a flexible schedule with no nights, no weekends, and no on-call, plus mileage reimbursement and full benefits for full-time staff. Tuition reimbursement is available for LPN/RN advancement.

Qualifications

• Active CNA certification required.
• Previous CNA experience; hospice/home health preferred.
• Valid driver's license and reliable transportation.
• Ability to pass background check and MVR screening.

Responsibilities

• Provide personal care, bathing, grooming and comfort per care plan.
• Observe, document and report changes in patient condition to supervisor.
• Support patients and families with compassion, dignity and respect.
• Assist with mobility, positioning and daily living activities safely.
• Document all care provided in the electronic medical record.

Qualifications: Key Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
