Job summary

Heritage-Woods-of-Minook is a skilled nursing facility seeking a compassionate CNA to support residents with daily living activities and person-centered care. You will assist with ADLs, promote independence, and document changes in health status for the clinical team.

Qualifications

• High School diploma or GED required.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity.
