Responsibilities

• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, toileting, and transfers.
• Provide hands-on patient care in accordance with individualized care plans.
• Take and record vital signs accurately.
• Observe and report changes in physical or mental condition to RN/LVN staff.
• Maintain accurate documentation in compliance with state and federal regulations.
• Ensure a clean, safe, and supportive environment for residents.
