Job summary

Wellsential Health in Rio Grande City, TX is seeking a Certified Nurse Aide to provide compassionate, hands-on care for residents on the 6pm–6am shift. You will assist with ADLs, monitor residents, and document care plans under supervision.

The role emphasizes safety, timely reporting, and teamwork with RN/LVN staff to uphold quality of life. Active Texas CNA (TULIP) certification and completion of a state CNA program are required; eligibility to work in the U.S. is essential.

Qualifications

• Active Texas CNA certification (TULIP) required.
• Completion of a state-approved CNA program.
• Must be eligible to work in the United States.

Responsibilities

• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, toileting, and transfers
• Provide hands-on patient care in accordance with individualized care plans
• Take and record vital signs accurately
• Observe and report changes in physical or mental condition to RN/LVN staff
• Maintain accurate documentation in compliance with state and federal regulations
• Ensure a clean, safe, and supportive environment for residents
