Job summary

Wyandot Memorial Hospital is seeking a certified Medical Records Coder to join our Medical Records team. This full-time role codes and abstracts patient encounters, reviews documentation for accuracy, and ensures compliant claims submission in collaboration with providers and Patient Financial Services.

The ideal candidate will hold CPC or CCS certification (or obtain within 12 months), possess strong attention to detail, and have experience with hospital/professional claims and Epic.

Qualifications

The Certified Medical Records Coder position is part of the Medical Records department and is a full-time role working approximately 80 hours per two-week pay period. This position is responsible for accurately coding and abstracting patient encounters to support compliant billing, reimbursement, and reporting. The Certified Medical Records Coder reviews clinical documentation, assigns diagnostic and procedural codes, audits coded data, and works collaboratively with providers and Patient Financial Services to ensure accurate and timely claims submission. This role requires strong attention to detail, knowledge of coding guidelines, and a commitment to confidentiality, quality, and customer service.

Qualifications

• High school diploma or equivalent required.
• Active CPC certification through AAPC or CCS certification through AHIMA required, or the ability to obtain certification within 12 months of hire.
• Knowledge of medical terminology required.
• Experience coding hospital and/or professional claims required.
• Inpatient or Outpatient surgery coding experience preferred.
• Epic experience preferred.
• Strong attention to detail and accuracy.
• Knowledge of medical coding guidelines, reimbursement requirements, and coverage determinations.
• Strong organizational, analytical, and problem-solving skills.
• Excellent written and verbal communication skills.
• Ability to maintain confidentiality and work effectively with providers, patients, payors, and other departments.

Key Responsibilities

• Code and abstract patient encounters, including diagnostic and procedural information and significant reportable elements.
• Review clinical documentation to ensure coded data accurately supports services provided and reimbursement requirements.
• Assign appropriate diagnostic and procedural codes in accordance with established coding guidelines and regulatory requirements.
• Audit clinical documentation and coded data for accuracy, completeness, and compliance.
• Review charges and documentation for appropriateness and identify additions to the fee schedule based on new procedures.
• Monitor compliance with local and national coverage determinations.
• Work closely with providers to obtain accurate and timely documentation for claims submission.
• Collaborate with Patient Financial Services to resolve coding and billing issues and ensure compliant claims submission.
• Maintain confidentiality and provide professional service to patients, providers, and internal departments.
• Perform other duties as assigned.
