Job summary

Advocate Health is seeking a qualified Home Health Aide to provide compassionate patient care under the supervision of an RN and the patient care plan. The role includes assisting with daily activities, maintaining patient safety, and documenting care in accordance with agency and regulatory standards.

Candidates must hold CNA certification, be in good standing, and obtain BLS within 6 months. A valid driver’s license is required for travel within the service area.

Qualifications

• Must be certified as a nurse assistant in the state where you work.
• Nurse assistant in good standing with the State Nurse Aide Registry.
• BLS certification within 6 months of hire.
• Valid driver’s license.

Responsibilities

• Contributes to effective patient outcomes; provide competent patient care as assigned by RN and care plan.
• Performs personal care needs including feeding, laundry, light cleaning of patient room. Maintains safety with patient handling.
• Communicates effectively in written and verbal documentation per agency, state, federal, and accrediting standards within required time frames.
• Demonstrates professionalism and cooperates with all associates; flexible with scheduling.
• Supports department and other service offices with scheduling as needed.
• Maintains home health aide skills through self-direction and ongoing training.
• Contributes to team, office, company and organizational goals; supports CQI processes.
• Assists with problem identification and solutions; participates in quality improvement projects.
• Demonstrates measures to ensure patient satisfaction; adheres to regulatory requirements.
