Job summary

Grace Healthcare Services in Ocean County, NJ is seeking a Certified Home Health Aide (CHHA) for a full-time position. Hours are 7:00am - 3:30pm with possible flexibility and weekend hours.

You will provide personal care, meal prep, and household assistance, documenting care with Celltrak and coordinating with RN Case Managers. Requirements include an active NJ CHHA in good standing, fluency in English, ability to lift up to 50 lbs, and willingness to travel within Ocean County.

Qualifications

• Valid NJ Certified Home Health Aide (CHHA) in good standing.
• Proficient in reading, writing, and communicating fluently in English.
• Ability to lift, push, and pull up to 50 lbs.
• Willingness to travel within Ocean County.

Responsibilities

• Personal Care: Assist patients with bathing, grooming, dressing, skincare, and mobility.
• Meal Preparation: Plan and prepare nutritious meals per dietary needs.
• Household Assistance: Maintain a clean, safe home environment for patients.
• Documentation: Record patient care activities using Celltrak.
• Team Collaboration: Communicate changes in patient conditions to RN Case Managers.
