Job summary

Elara Caring is seeking a Hospice CNA/HHA to join our compassionate home-health team. You will verify and implement the Plan of Care, collaborate with RNs and physicians, document medications, and provide direct patient care in patients' homes across the community.

This role requires 1 year of hospice aide experience, a valid driver's license, and the ability to lift up to 50 pounds. We offer competitive pay and a comprehensive benefits package, with opportunities for advancement.

Qualifications

  • High School Diploma or GED is required.
  • 1 year of Hospice Aide/Nursing Assistant experience is required.
  • Must complete hospice aide training per 42 CFR 484.36 and competency evaluation program.
  • Ability to lift up to 50 pounds and travel 50%.
  • Valid driver’s license and reliable transportation to perform job tasks.

Responsibilities

  • Verifies the Plan of Care and provides care per RN orders and established standards.
  • Incorporates patient care goals into nursing visits with documentation in notes.
  • Communicates findings/changes to RN Case Manager, physician, and supervisor.
  • Reports unsafe conditions to RN Case Manager/physician.
  • Implements patient safety plan using patient, family, and community resources.
  • Assists patient with self-care techniques while delivering quality care.
  • Suggests revisions to the plan of care and notifies RN Case Manager.
  • Documents medications and updates medication profile.
  • Collaborates with RN Case Manager during business hours for guidance.
  • Provides supportive communication to patients, families, and staff.

As a Hospice C.N.A /H.H.A., you’ll contribute to our success in the following ways:
  • Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines.
  • Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes.
  • Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.
  • Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate.
  • Implements the plan for patient safety, using the patient, family, and community resources.
  • Assists the patient in learning appropriate self-care techniques while delivering quality patient care.
  • Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.
  • Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
  • Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.
  • Documents medication regimen and updates medication profile according to established procedures.
  • Treats patients and caregivers in the highest and most effective manner.
  • Implement all available actions to prevent avoidable hospitalizations and ER visits.
  • Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
What is Required?
  • High School Diploma or GED equivalent required
  • 1 year of experience as a Hospice Aide or Nursing Assistant is required
  • State-established or other Hospice Aide training program that meets the requirements of 42 CFR
  • 484.36 and a competency evaluation program
  • Experience in a home health environment is preferred
  • 50% travel required
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
