Job summary

Elara Caring in Dallas, TX is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join our patient-centered home care team. You’ll provide skilled nursing care under the plan of care, support families, and collaborate with the RN Case Manager to ensure comfort and safety for patients in their homes.

Requirements include a high school diploma or GED, 1 year in hospice or nursing assistant role, valid driver's license or reliable transport, and the ability to lift up to 50 pounds.

Qualifications

  • High School Diploma or GED equivalent required.
  • 1 year of experience as a Hospice Aide or Nursing Assistant is required.
  • State-established Hospice Aide training program meeting 42 CFR 484.36 is required.
  • Must be able to frequently lift, push, pull, and move up to 50 pounds.
  • Valid driver’s license or reliable transportation to travel between patient homes.
  • Home health experience is preferred.

Responsibilities

  • Provide skilled nursing care according to the Plan of Care, physician orders, and RN direction.
  • Monitor patient condition and report changes to the RN Case Manager and care team.
  • Collaborate with the RN Case Manager to update the Plan of Care as needed.
  • Educate patients and caregivers on self-care techniques to support safety and independence.
  • Document medication regimens and maintain accurate profiles.
  • Help prevent avoidable hospitalizations through proactive, coordinated care.
  • Maintain compassionate, high-quality care with clear communication to patients, families, and physicians.

Job Description

As a Hospice C.N.A /H.H.A., you’ll contribute to our success in the following ways:

  • Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
  • Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
  • Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
  • Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
  • Document medication regimens and maintain accurate, up-to-date medication profiles.
  • Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
  • Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.
What is Required?
  • High School Diploma or GED equivalent required
  • 1 year of experience as a Hospice Aide or Nursing Assistant is required
  • State-established or other Hospice Aide training program that meets the requirements of 42 CFR
  • 484.36 and a competency evaluation program
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
  • Experience in a home health environment is preferred
