Job summary

Apple Rehab Farmington Valley is seeking a CNA to join our 130-bed skilled nursing and rehabilitation center on Farmington Avenue in Plainville, CT. You will assist residents with daily bathing, grooming, mobility and personal care, helping maintain comfort and dignity in a warm, family-like setting.

We offer full-time 7-3, part-time 7-3 and 11-7, and per diem shifts with weekend requirements. Benefits include medical/dental/vision, 401K, generous PTO and holiday pay, life and disability

Qualifications

• Must hold current CNA certification in good standing.
• No history of patient/resident abuse as per State Registry.
• Proficient in reading, writing, and following oral and written instructions.
• Strong attention to detail, excellent follow-through, and ability to prioritize multiple tasks.
• Team player with good cooperation and interpersonal skills.
• Demonstrates patience, tact, enthusiasm, and a positive attitude towards residents.

Responsibilities

• Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use.
• Documents personal care given according to facility policies and reports changes to supervisor.
• Reports accidents or injuries immediately and records charting entries.
• Answers call lights and responds to emergencies calmly and efficiently.

Differentials Offered:

• Shift Differentials for 2nd and 3rd shifts and weekends
• Additional Shifts Differential: Extra $5.00/hour for any shifts worked outside scheduled hours

Available Shifts:

• Full-Time 7-3
• Part-Time 7-3 and 11-7

Responsibilities:

Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use, positioning in and out of bed. C.N.A. Certified Nurse’s Assistant documents personal care given according to facility policies. Provides nursing functions as directed including perineal care, making beds, turning residents, measures and records temperature, weight, pulse and respiration. C.N.A. Certified Nurse’s Assistant observes and reports to supervisor changes in residents’ condition, including any skin breakdowns. Reports immediately all accidents or injuries. Records all necessary charting entries. Answers all call lights regardless of assignment. Responds to emergency situations calmly and efficiently.

Per diem staff are expected to fulfill (2) weekend shifts monthly to meet requirements.
