Oak Street Health is seeking a Center Nurse to support patient care within a value-based care model. The role focuses on assessments, chronic condition management, and collaboration with the care team to reduce hospitalizations.

You will perform wound care, administer vaccines, conduct patient education, and drive safety through standardized protocols while coordinating with providers and staff to maintain high-quality care.

Qualifications

• Active, unrestricted RN licensure in the state you reside.
• BLS or CPR certification required.
• Fluent written and spoken English.
• US work authorization.
• Experience with Electronic Medical Records.
• Ability to maintain patient confidentiality.
• Ability to assess patients without face-to-face interaction.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management.
• Perform nursing procedures including wound care and vaccines.
• Educate patients and caregivers about chronic conditions and prevention.
• Review medications for safety and adherence.
• Conduct onsite testing and use data to close gaps in screenings.
• Coordinate care plans with providers and the interdisciplinary team.
• Manage medication renewals and prior authorizations.
• Ensure timely clinical communications and complete patient encounters.
• Delegate tasks to medical assistants and phlebotomists.
