Job summary

Qualifications

• Active, Unrestricted RN licensure in the state
• BLS or CPR certification
• Fluent written and spoken English
• US work authorization
• Electronic Medical Record experience
• Ability to maintain patient confidentiality
• Ability to assess patients without face-to-face interaction

Responsibilities

• Support patient assessment and triage to ensure appropriate level of care
• Perform wound care and medication administration, including vaccines
• Conduct medication reviews and coordinate with care team
• Onsite testing (point-of-care labs, EKGs) and data review to close care gaps
• Coordinate weekly transitions from hospital to clinic with providers
• Delegate tasks to medical assistants and phlebotomists as needed
• Maintain timely clinical communications and patient encounters closure
• Engage in ongoing professional development and compliance with safety protocols
