• BLS certification required within 90 days.

Responsibilities

• Independently provides care for older adults with acute and chronic illnesses.
• Leads care teams and ensures patient care quality and outcomes.
• Engages hospitalists and coordinates with case managers and specialists.
• Drives center clinical operations, patient access and member satisfaction.
• Mentors PCPs and participates in recruiting and interviewing providers.
