Job summary

Responsibilities

• Supervise and train primary care providers (PCPs).
• Maximize core model execution, improving clinical quality, efficiency, outcomes, and satisfaction.
• Provide direct patient care as a PCP and/or other clinical duties.
• Lead care teams, including medical assistants and care coordinators.
• Engage with hospitalists and transitional care teams.
• Lead Super Huddles and Transforming Care Meetings.
• Manage, mentor, and coach PCPs for outstanding clinical outcomes.
• Partner with Center Operations Director/Market General Manager to improve financial performance and increase membership.
