Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
• Current, active MD licensure in the state of employment.
• Minimum of 1‑year clinical experience in geriatric, adult, or family practice setting.
• Board certification preferred; board eligibility required.
• Current DEA number for schedule II‑V controlled substances.
• BLS certification from AHA or Red Cross within 90 days of employment.

Responsibilities

• Independently provide care for older adult patients with acute and chronic illnesses.
• Lead the care team, including medical assistant and care coordinator.
• Engage with hospitalists for admitted patients and coordinate care.
• Perform assessment, diagnosis, treatment, and documentation for complex patients.
• Lead SH and Transforming Care Meeting weekly and support center leaders.
• Recruit patients and interview additional providers for the company.
