Job Summary:

The Care Coordinator is responsible for supporting our members as they navigate the complex healthcare delivery system, ensuring every member receives the right care in the right setting. This individual will serve as a guide for the member and their families to ensure the member meets with the right specialists, inside or outside the center, in a timely manner. An ideal candidate will have a deep understanding of the healthcare ecosystem, strong attention to detail, and be driven by a passion for providing quality care to seniors.

Duties/Responsibilities:
  1. Partner with ArchWell Health's multi-disciplinary care team to understand each member's care needs, including identifying gaps and barriers to treatment.
  2. Coordinate referrals to external providers, vendors, and services to ensure care needs are met.
  3. Complete proper documentation for member referrals to ensure records are accurate and current.
  4. Build and nurture relationships with members and their families to facilitate care coordination.
  5. Carry out activities and provide ongoing monitoring as outlined in a member's care plan to ensure routine follow-up and progression on care plan goals.
  6. Adhere to HIPAA standards and maintain confidentiality of protected health information (PHI).
  7. Promote clear communication among care team members and treating clinicians to ensure positive outcomes.
  8. Assist members in resolving issues related to the healthcare system, including financial or social barriers (e.g., requesting interpreters, transportation services, or prescription assistance).
  9. Serve as the system navigator and point of contact for members and families, providing direct access for questions and concerns.
  10. Additional duties as assigned.
Required Skills/Abilities:
  • Deep understanding of the healthcare ecosystem, preferably with experience in benefits and insurance authorization.
  • Passion for helping senior members successfully carry out care plans.
  • Effective collaboration with diverse populations, including staff, providers, members, families, insurance carriers, vendors, and the public.
  • Basic medical terminology knowledge.
  • Understanding of Medicare criteria for equipment and supplies.
  • Knowledge of CPT and ICD-10 coding systems.
  • Familiarity with community resources and specialists to assist members.
  • Strong communication and rapport-building skills within an interdisciplinary team.
  • Ability to multitask and manage competing priorities with high attention to detail.
  • Proficiency in PC skills, including Microsoft Office.
  • Fluency in Spanish or other languages spoken in the communities served, where necessary.
Education and Experience:
  • High School Diploma or equivalent required; college coursework in healthcare preferred.
  • At least 2 years of experience in referral management or care coordination.
  • Experience as a Medical Assistant or in medical administration is helpful.
  • Experience with EMR systems, preferably eClinicalWorks or similar.
