Akkodis is seeking a compassionate and detail-oriented Care Coordinator for a temporary 4-month remote contract. In this role, you will work directly with members to educate, support, and guide them through their healthcare journeys. You will assist in addressing barriers to care, connecting members with community resources.

Employment type: Temporary Contract (4 Months)

Location: Remote

Pay: $28/hr

Essential Functions:

  • Conduct initial and ongoing outreach to members, ensuring they understand and follow their care plans.
  • Assist members in identifying, completing, and obtaining required documents, tests, and items needed for medical appointments.
  • Identify and address barriers preventing members from accessing and attending medical appointments, including arranging transportation, durable medical equipment (DME), and translation services.
  • Collaborate with primary care physicians and care management staff to identify members requiring specialty care services.
  • Refer and connect members to appropriate community resources and support services.
  • Document and maintain all activities and records in the appropriate system(s) following established standards.

Education and Experience:

Required:

  • High school diploma or equivalent.
  • Minimum of two years of experience in a related role, such as community health advocate, social worker, patient advocate, or patient coordinator.
