Job Summary:

The Care Coordinator supports members in navigating the complex healthcare system, ensuring they receive appropriate care in the right setting. This role involves guiding members and their families to meet with the right specialists, either within or outside the center, in a timely manner. An ideal candidate will have a deep understanding of the healthcare ecosystem, strong attention to detail, and a passion for providing quality care to seniors.

Duties/Responsibilities:
  1. Partner with ArchWell Health's multidisciplinary care team to understand each member's needs, identifying gaps and barriers to treatment.
  2. Coordinate referrals to external providers, vendors, and services to meet care needs.
  3. Complete accurate and current documentation for member referrals.
  4. Build and nurture relationships with members and their families to facilitate care coordination.
  5. Monitor ongoing activities as outlined in each member's care plan to ensure progress on goals.
  6. Adhere to HIPAA standards and maintain confidentiality of protected health information (PHI).
  7. Promote clear communication among the care team and clinicians to ensure positive outcomes.
  8. Assist members with problem-solving related to healthcare, financial, or social barriers (e.g., interpreters, transportation, prescriptions).
  9. Serve as the system navigator and point of contact for members and families, providing direct access for questions and concerns.
  10. Perform additional duties as assigned.
Required Skills/Abilities:
  1. Deep understanding of the healthcare ecosystem, including benefits and insurance authorization.
  2. Passion for helping senior members successfully carry out care plans.
  3. Effective collaboration with diverse populations, including staff, providers, members, families, insurance carriers, and vendors.
  4. Basic medical terminology knowledge.
  5. Understanding of Medicare criteria for equipment and supplies.
  6. Knowledge of CPT and ICD-10 codes.
  7. Working knowledge of specialists and community resources.
  8. Strong communication and rapport-building skills within an interdisciplinary team.
  9. Ability to multitask and manage priorities with high attention to detail.
  10. Proficiency in Microsoft Office.
  11. Fluency in Spanish or languages spoken by the community served (where necessary).
Education and Experience:
  1. High School diploma or equivalent required; college coursework in healthcare preferred.
  2. At least 2 years of experience in referral management or care coordination.
  3. Medical Assistant or administrative experience is helpful.
  4. Experience with EMR systems, ideally eClinicalWorks or similar.
