Job summary

San Francisco Post Acute in San Francisco, CA, is seeking an experienced Admissions Director to lead admissions operations, grow facility census, and coordinate a smooth resident admission experience for families and residents.

You will build referral relationships with hospitals, physicians, social workers, and discharge planners, ensure timely documentation and insurance verification, and partner with DON and clinical teams to optimize admissions while upholding high service standards.

Qualifications

• Knowledge of admissions processes, insurance verification, medical record review, and resident intake procedures.
• Strong interpersonal, communication, negotiation, and customer service skills.
• Excellent organizational skills with the ability to manage multiple referrals and admissions simultaneously.
• Proficiency with computers, electronic medical records, and standard office applications.

Responsibilities

• Manage, direct, and monitor daily admissions operations to meet budgeted census goals.
• Coordinate the admission process from initial referral through resident arrival and paperwork completion.
• Screen prospective residents by reviewing medical records, clinical needs, and admission criteria.
• Develop and maintain referral relationships with hospitals, physicians, social workers, and discharge planners.
• Create a welcoming admission experience and ensure timely communication with residents, families, and staff.
