NorthStar Behavioral Health in Cincinnati, OH is hiring a Care Coordinator for the Adult Residential Care Program (ARC).

This full-time role involves clinical case management for adults with substance use disorders and mental health needs, delivering evidence-based, person-centered interventions, completing assessments within 24 hours, and coordinating with providers to ensure continuity of care in the community.

Qualifications

• High School Diploma or GED, Associate Degree Preferred.
• Entry-level experience working with SUD Population.
• Current Ohio Chemical Dependency Counselor Assistant (CDCA) - Preliminary acceptable.
• Eligible to provide services and maintain active status with Ohio Medicaid and all applicable third-party and billing entities.
• Valid Ohio driver’s license and insurance.
• Ability to pass a background check and drug screen.
• Tuberculosis Skin Test and Influenza (Flu) vaccination within the past year.
• Ability to work a shift of 8:30 a.m. – 5:00 p.m. (M-F).

Responsibilities

• Provides clinical therapeutic interventions and case management services to adults with substance use disorders and/or psychiatric disabilities.
• Utilizes evidence-based, person-centered approaches to assess needs, develop individualized service plans, coordinate care, and support clients in improving psychosocial functioning.
• Completes case management needs assessments within 24 hours of admission to identify client needs and functioning.
• Delivers care coordination, case management, treatment planning, and documentation to support recovery.
• Develops individualized service plans using assessment findings and coordinates interventions.
• Facilitates groups to support recovery and coping skills.
• Provides crisis intervention and de-escalation services.
• Allocates at least 55% of time to direct clinical care.
• Completes all clinical and administrative documentation within 48 hours of service delivery.
• Attends weekly supervision and team meetings as required.
