Job summary

Lighthouse Behavioral Health Solutions in Lancaster, OH is seeking a Case Manager (CDCA) to assess client needs and develop comprehensive care plans for SUD and MH clients. You will report to a Case Management Supervisor and coordinate services to promote recovery and engagement in the community.

Qualifications

• CDCA certification required.
• State of Ohio driver's license and active insurance.
• Must pass BCI check, corporate compliance checks, and employment drug screen.

Responsibilities

• Coordinates and provides safe, timely, effective, efficient, equitable, culturally sensitive, and client-centered care.
• For SUD clients: handles case assignments, assesses barriers to recovery in various life domains, drafts service plans and reviews progress.
• For MH clients: coordinates mental health treatment planning with CPST goals and collaborates with clinicians on treatment plans.
• Evaluates daily living functioning and helps clients set goals to improve daily living.
• Completes billing and documentation accurately and timely.
• Sees clients weekly or per level of care and ensures HIPAA and confidentiality compliance.
• Provides linkage to inpatient or more restrictive treatment as needed and coordinates with community resources.
