Job summary

Leora Behavioral Health seeks a dedicated Case Manager (CDCA) in Dayton, OH to coordinate access to housing, food, and mental health services, and to support clients through treatment plans. You will document interactions, assist with billing, and maintain comprehensive client files.

The role requires CDCA certification in Ohio, at least 1 year of related experience, and being 21 or older. This is a full-time, Monday–Friday position with pay range $19–$22 per hour.

Qualifications

• At least 1 year of experience in duties described.
• Must be certified as a CDCA in the State of Ohio.
• Be 21 or older.

Responsibilities

• Facilitate access to community resources for clients.
• Coordinate and monitor services and treatment plans.
• Document client encounters and billing; maintain files.
• Identify and provide emergency crisis services as needed.
• Assess client progress and participate in case reviews.
• Maintain program statistics.
