This role supports adults with substance use disorders and psychiatric disabilities through clinical interventions, case management, and resource linkage. A CDCA certification is preferred, with valid Ohio license and background screening required.

Qualifications

• High School Diploma or GED; Associate degree preferred.
• Entry-level experience working with SUD population.
• CDCA certification or eligibility to obtain Ohio Medicaid status.
• Valid Ohio driver’s license and auto insurance.
• Ability to pass a background check and drug screen.
• TB Skin Test and Flu vaccination within past year.

Responsibilities

• Provides clinical therapeutic interventions and case management services to adults with SUD and/or psychiatric disabilities.
• Uses evidence-based, person-centered approaches to assess needs and develop service plans.
• Completes needs assessments within 24 hours of admission.
• Delivers care coordination, treatment planning, and documentation.
