Qualifications

Responsibilities

• Plans, implements, and evaluates patient care based on assessment to optimize outcomes.
• Monitors, documents, and communicates patient condition; delegates care to staff.
• Precepts students and new hires; educates patients and families as appropriate.
• Coordinates care across disciplines, including disposition, referrals, and psychosocial support.
