Job summary

Bedford-Somerset DBHS is seeking County Caseworkers to build rapport with consumers and advocate for needed services. You will monitor services, develop individualized plans, and coordinate with community resources. Hybrid of remote and on-site work is available after probation, with full-time hours 8:00-16:00.

The role focuses on connecting people to essential resources and ensuring progress while following state regulations and agency policies.

Qualifications

• Two years of experience as a County Social Services Aide 3 or equivalent in social services work.
• Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or related field.

Responsibilities

• Monitor, link, and evaluate services and make referrals as needed.
• Provide direct face-to-face case management with productivity in mind.
• Develop individualized service plans with involvement of the consumer and/or family.
• Enter information into state computer systems and maintain records.
