Job summary

Commonwealth of Pennsylvania seeks County Caseworker 1 to support individuals with developmental delays, mental health needs, or intellectual disabilities by linking them to essential resources and tracking progress. The role focuses on building rapport, delivering services, and promoting self-determination within a county framework.

You will monitor services, conduct direct contact, and collaborate with agencies to create individualized plans.

Qualifications

• Two years of experience as a County Social Services Aide 3 and two years of college coursework including 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences; or
• A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences; or
• Any equivalent combination of experience and training and one year of paraprofessional case management experience.

Responsibilities

• Service Monitoring: monitor, link, and evaluate services; refer to other services as needed.
• Direct Contact: perform face-to-face case management with individuals and families.
• Plan Development: develop an individualized service plan with involvement of the individual and/or family.
• Information Management: enter information and maintain records in databases and charts per regs and policy.
• Crisis Support: provide crisis intervention and coordinate assistance when needed.
• Collaboration: work with staff, providers, and community resources.
• Transportation Support: link or transport individuals to appointments as needed.
