Job summary

You will connect people with essential resources and track progress, while advocating for self-determination and coordinating services.

Responsibilities include monitoring services, direct contact with clients, developing individualized service plans, and maintaining accurate records in state systems.

Qualifications

• Six months of experience as County Caseworker 1 or equivalent.
• Completion of County Social Casework Intern program.
• Bachelor’s degree with social welfare major or related.
• Combination with 12 credits in social sciences and one year of related work.
• Applicant may meet requirements within two months of graduation.

Responsibilities

• Monitor, link, and evaluate services; refer to other services as needed.
• Provide regular direct contact and face-to-face case management.
• Schedule and facilitate plans with input from individual and family.
• Enter and maintain data in state systems and charts per regulations.
• Offer crisis intervention and coordinate assistance when required.
• Collaborate with agency staff, providers, and community resources.
• Link individuals to transportation or arrange transport as needed.
