Job summary

The Commonwealth of Pennsylvania's Bedford-Somerset DBHS is seeking a County Caseworker 2 to support individuals with developmental delays, mental health needs, or intellectual disabilities. You will monitor services, link resources, and help families develop supports in a compassionate, community-focused environment.

You will conduct direct case management, maintain records in state systems, attend meetings, and collaborate with community partners.

Qualifications

• Six months of experience as a County Caseworker 1.
• Bachelor’s degree with social welfare major or equivalent credits.
• Ability to meet educational credits within two months of graduation.
• Experience linking services and case management preferred.

Responsibilities

• Monitor, link, and evaluate services and referrals.
• Provide direct face-to-face case management to individuals.
• Develop individualized service plans with individuals/families.
• Enter data into state computer systems and maintain charts.
• Coordinate crisis intervention and connect to resources.
• Collaborate with agency staff and community resources.
• Arrange transportation to appointments as needed.
