Job summary

The Commonwealth of Pennsylvania’s Northumberland County Behavioral Health & Intellectual/Developmental Services Program seeks a County Caseworker 2 to help individuals gain independence by guiding access to resources, building daily life skills, and staying connected with essential programs.

You will develop personalized service plans, assist with benefits applications, monitor services, document care, coordinate progress, and respond to crises under supervision.

Qualifications

• Six months of experience as a County Caseworker 1
• Completion of County Social Casework Intern program
• Bachelor's degree with a social welfare major
• Bachelor's degree with 12 relevant credits and one year of related social casework experience
• Any equivalent combination of experience and training with 12 credits in social sciences
• Within two months of graduating to meet educational requirements

Responsibilities

• Plan Development: Prepare individualized service plans outlining goals and supports
• Benefit Assistance: Guide applications for financial and medical benefits
• Provider Monitoring: Review services to ensure delivery of planned supports
• Care Documentation: Maintain records supporting care, life planning, and advocacy
• Progress Coordination: Lead meetings to evaluate progress toward goals
• Crisis Intervention: Respond to emergencies as assigned
