Job summary

Summit Pointe is seeking home-based caseworkers to serve youth and families in Southwest Michigan. You will work with a skills coach and therapist to implement tailored strategies and meet treatment goals.

Responsibilities include psychosocial assessments, coordination, and safety planning within agency guidelines. A bachelor’s degree in human services and some computer experience are required or preferred. Benefits are included.

• Bachelor’s degree in a human services field required.
• One year of computer experience with Word/Excel preferred.
• Trauma-informed approach training is expected.

Job Summary/Objective:Homebased services are a community-based therapeutic service who work with youth and families with several emotional disturbances. Services are provided within the community. Homebased caseworkers are members of a small team comprised of a skills coach and therapist who work together to impact goals listed on the youth’s treatment plan.Essential Duties and Functions:Implement a customized set of strategies, supports and services to meet the individualized goals of the youth and family.Completion of psychosocial assessments.Coordination and monitoring.Education and training.Engages in risk management and completes safety planning.Performs outreach activities within the community.Maintains record compliance according to agency guidelines and standards as well as funding source regulations.Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements or a minimum of 24 hours per year of clinically based trainings), implementation of evidence-based practices, peer review attendance, and professional improvement practices.Qualifications:Bachelor’s degree in a human services field is required.
