Job summary

Southern Highlands Community Mental Health Center in McDowell County, West Virginia is seeking a Case Manager/Care Coordinator Level 2 for our Outpatient Services team. This role supports clients and families in accessing services to promote stability and well-being.

The ideal candidate will coordinate care with treatment providers and community partners, advocate for client needs, and maintain professional boundaries while delivering person-centered, confidential services.

Qualifications

• Bachelor’s or Master’s degree in a human-services-related field per WV Board of Social Work requirements; or
• Licensed psychologist with master’s or doctoral degree; or
• Licensed registered nurse.

Case Manager/Care Coordinator Level 2 – Outpatient Services

Position Summary

Southern Highlands Community Mental Health Center is seeking a compassionate, dependable, and organized Case Manager/Care Coordinator Level 2 to join our Outpatient Services team in McDowell County.

This position assists clients and families in accessing the services and resources needed to promote stability, independence, and overall well-being. The Case Manager/Care Coordinator works closely with clients, families, treatment providers, and community organizations to coordinate care, advocate for client needs, and support progress toward individualized treatment goals.

Education and Licensure Requirements

Applicants must meet at least one of the following qualification pathways:

• Bachelor’s or master’s degree in a human-services-related field that meets the requirements established by the West Virginia Board of Social Work;
• Licensed psychologist with a master’s or doctoral degree from an accredited program; or
• Licensed registered nurse.
