Job summary

Southern Highlands Community Mental Health Center in McDowell County, West Virginia is seeking a Case Manager/Care Coordinator Level 2 for our Outpatient Services team. This role supports clients and families in accessing services to promote stability and well-being.

The ideal candidate will coordinate care with treatment providers and community partners, advocate for client needs, and maintain professional boundaries while delivering person-centered, confidential services.

Qualifications

• Bachelor’s or Master’s degree in a human-services-related field per WV Board of Social Work requirements; or
• Licensed psychologist with master’s or doctoral degree; or
• Licensed registered nurse.

Responsibilities

• Coordinate care with clients, families, treatment providers and community organizations.
• Advocate for client needs and support progress toward individualized treatment goals.
