Qualifications

• Valid driver’s license and reliable transportation.
• Ability to pass NCIC/CIB and APS/CPS background checks.
• Three professional references.
• 2–4 years of case management and/or behavioral health experience preferred.
• 1–2 years of experience leading teams preferred.
• Experience providing direct services to children and families preferred.

Responsibilities

• Provide wraparound services to a caseload of up to 15 families.
• Coordinate supports, resources, and services using a strengths-based approach.
• Facilitate team meetings with youth, families, and multidisciplinary partners.
• Conduct and administer the Child and Adolescent Needs and Strengths (CANS) assessment.
• Maintain accurate, timely, and compliant documentation.

Key Responsibilities

• Provide wraparound services to a caseload of up to 15 families
• Coordinate supports, resources, and services using a strengths‑based approach
• Facilitate team meetings with youth, families, and multidisciplinary partners
• Collaborate with MCO care managers and community resources
• Conduct and administer the Child and Adolescent Needs and Strengths (CANS) assessment
• Monitor service implementation and ensure families receive identified supports
• Lead planning from engagement through transition and discharge
• Conduct home visits according to each family’s individualized plan
• Identify service gaps and collaborate with community providers to address needs
• Maintain accurate, timely, and compliant documentation
• Participate in scheduled supervision and coaching
• Maintain client records in alignment with agency standards and regulatory guidelines

Physical Requirements

• Ability to walk up and down stairs
• Ability to lift 5–50 pounds
• Ability to kneel, bend, squat, and stand
• Ability to operate a motor vehicle and follow all state traffic laws
