Home Base Inc. – Statewide (West Virginia) is seeking a Wraparound Facilitator/Case Manager for our Behavioral Health Program. This full-time role supports children and families with hands-on experience in a reputable organization.

Qualifications

• Bachelor’s Degree in a human services field required.
• Experience or interest in wraparound/behavioral health preferred.
• Ability to pass background checks (NCIC/CIB and APS/CPS) and maintain clear records.

Responsibilities

• Provide wraparound services to a caseload of up to 15 families.
• Coordinate supports and services using a strengths-based approach.
• Lead team meetings with youth, families, and multidisciplinary partners.
• Collaborate with MCO care managers and community resources.
• Conduct and document the CANS assessment.
• Monitor service implementation and ensure identified supports are delivered.
• Lead planning from engagement through transition and discharge.
• Conduct home visits according to each family’s plan.
• Identify service gaps and advocate with providers to address needs.
• Maintain accurate, timely, and compliant documentation.
• Participate in supervision and coaching and maintain client records.

Key Responsibilities

• Conduct home visits according to each family’s individualized plan
• Identify service gaps and collaborate with community providers to address needs
• Maintain accurate, timely, and compliant documentation
• Participate in scheduled supervision and coaching
• Maintain client records in alignment with agency standards and regulatory guidelines

Physical Requirements
